Objective:To examine the effect of sildenafil (Viagra) on the blood pressure (BP) response in men with erectile dysfunction (ED) treated with sildenafil.

Method:The study included 696 patients with ED treated with sildenafil and 536 men without ED who were included in the study. BP was measured at the beginning of the ED treatment and at 6 and 12 months after therapy, and at follow-up.

Results:Of the 696 patients, 855 (77.9%) took sildenafil; 751 (59.3%) took sildenafil and 446 (31.3%) took sildenafil and 536 (33.3%) took sildenafil. The mean age of the patients was 48.7 ± 13.1 years.

Conclusions:A significant increase in mean systolic BP was noted in sildenafil and sildenafil-treated ED patients. This may indicate a possible positive effect on BP regulation and possibly may also be related to the treatment itself.

Viagra (sildenafil) - effect on blood pressure in men with erectile dysfunction (ED) treated with sildenafil.

ABSTRACT

The use of sildenafil, a phosphodiesterase type 5 inhibitor, has been associated with an increased risk of cardiovascular disease. In patients with ED, sildenafil may prevent exercise-induced pulmonary vasoconstriction and vasodilation that may be responsible for cardiovascular events. It also increases the sensitivity of the endothelium to nitric oxide. However, the effects of sildenafil on blood pressure in patients with ED remain controversial. Several randomized, controlled, double-blind, placebo-controlled trials have been performed on sildenafil and the effects of sildenafil on blood pressure in ED, including a pilot study that was terminated prematurely after one year. In this pilot study, sildenafil was studied in 473 patients with ED who were treated with sildenafil. The study showed a significant decrease in supine BP (mean: -0.6 mmHg; -0.9 to -0.6 mmHg; -1.4 to -0.6 mmHg; -0.9 to -0.9 mmHg) and an increase in supine systolic BP (mean: -1.0 mmHg; -1.0 to -1.2 mmHg; -0.7 to -1.8 mmHg; -1.4 to -1.5 mmHg; -0.7 to -0.9 mmHg; -0.9 to -0.9 mmHg; -0.9 to -1.0 mmHg; -0.7 to -0.9 mmHg) when compared to patients without ED. The change in supine systolic BP was not statistically significant.
